All employees working for a PERA-affiliated employer, as well as retirees who have returned to work for PERA-affiliated employers, are immediately eligible to join the Plan. There are no minimum service requirements to fulfill.
Enrolling in the PERAPlus 401(k) Plan is a two-step process. First, you must print out the 401(k) Contribution Authorization Form to give to your employer. Then, you must submit your initial asset allocation using this Web site.
Please note: This form is for new enrollments only. Do not use this form to make changes to your account. If you already have a PERA PIN/User ID and password for your account, any changes you make here will not be accepted. If you already have a PERAPlus 401(k) account and would like to change your asset allocation, go to the Account Access section of PERA's Web site. Enter your PERA PIN/User ID and password, then click on "401(k) Account Access," located on the right side of your screen or under the Inquiry menu.

You must print out the 401(k) Contribution Authorization Form and give it to your employer's payroll office. This form allows your employer to deduct contributions from your paycheck. If you do not give this form to your employer, you will not be enrolled in the PERAPlus 401(k) Plan. Do not send the form to Voya FinancialTM or PERA.
